Engineers repeatedly warned that the rocket's electronic control systems had not been fully tested. However, the relentless calendar pressure meant their safety concerns were brushed aside. On the day of the test, October 24, 1960, Launch Pad 41 at the Baikonur Cosmodrome was crowded with personnel. Approximately 250 people were present on the field, with many standing dangerously close to the fully fueled prototype.

The disaster began with a sudden fuel leak in the prototype R-16 rocket's fuel tanks. The highly corrosive mixture quickly spread to the lower section of the two-stage rocket. Almost instantly, 160 tons of nitrogen tetroxide and nitric acid propellant ignited, triggering a massive, violent explosion that engulfed the entire launch site in a massive fireball.

The blast and the resulting inferno killed 126 people on the spot. Among the victims was General Mitrofan Nedelin, the head of the R-16 project. The heat of the fire was so intense that many of the bodies were burned beyond recognition, and investigators had to identify the victims using personal belongings like keys and pocket watches found in the ashes.

For decades, the tragedy was completely erased from official history. The Soviet government did not acknowledge or confirm the accident until 1989, during the era of glasnost. Today, the tragedy stands as a sobering reminder of the human cost of political pressure and rushed technology in the race for global dominance.
